Overview
These are questionnaires that appear on your smart coach screen to tell us more about you to help your coach to help you with your care journey. Your answers will be available for your coach.
Once a questionnaire has been assigned to you, it will show on the smart coach screen labeled Questionnaire.
Answering questionnaire
- When you see a questionnaire appear, tap answer.
- Follow the prompts to answer the questions.
- When prompted, tap “Submit” at the bottom of the screen followed by Done at the bottom.
- Once you submitted you answer, the questionnaire will be saved to your Today screen
